Seniors and Caregivers Support Services
Legal Agreement

 

The Seniors and Caregivers Support Services Unit at Family Service Toronto (FST) receives part of its funding from the Toronto Central Local Health Integration Network (TC LHIN).  As part of this funding, FST is required to enter into a  Multi-Sector Service Accountability Agreement (M-SAA) with the TC LHIN.  This agreement outlines the nature of FST’s obligations to the TC LHIN.  

In the interests of full public accountability and transparency, and as one of the conditions of the M-SAA, FST is required to publish the fully executed legal agreement.  This agreement is provided below. Additional copies are posted in the office locations from which the Seniors and Caregivers Support Services Unit provides service (355 Church Street, 747 Warden Ave, 138 Pears Ave and 25 Leonard Ave).
